How to Create a Consent Template
Let’s take a look at how to create a consent template.
How to Use Templates in Consent On Demand
Templates help you save time and streamline your consent requests. Here’s how to create and use them.
Access Templates
Log into your account and click Templates in the left-hand menu.
New accounts start with no templates, so you’ll need to add your own.
Option 1: Create a Template from Scratch
- Click Add Template.
- Enter a template name (e.g., Project Agreement).
- Add and format your content:
- Edit text (bold, center, resize)
- Add headings or titles
- Insert custom fields (e.g., First Name, Last Name).
- Add form fields as needed:
- Text boxes (e.g., Company Name)
- Date fields (e.g., Project Start Date)
- Checkboxes (e.g., Agreement confirmation)
- Click Save.
Your template will now be available when sending consents.
Option 2: Use Preset Templates
- Click Add Preset Template.
- Select your industry (e.g., Insurance).
- Browse available forms (e.g., CMS forms, release forms).
- Select one or more templates and add them to your account.
Important: Preset templates may not meet all legal requirements. You are responsible for ensuring compliance with laws and regulations in your area.
Using Templates When Sending Consents
- When sending a consent, simply select a saved template.
- You can choose whether to share templates with your team or keep them private.
Editing Templates Anytime
You can edit templates at any time, make changes, and save updated versions. You can also edit the template on the fly for one off needs and NOT save the changes so your template will not be affected.
